Lead Form Plus Extras
I need to build a form to send leads to a client’s email address.

The form should be almost a replica of the one found on the left-hand side of this page, starting with “We Can Help”:

I will post this form on a blog post found on my website:

The main differences that should be incorporated into my form are:

1) Submit Form button should alternate between two grey-scale shades rather than gold;

2) The option “Video” should be inserted after “Internet Search” for the question, “How did you hear about this Web site?”;

3) The form should post to an email address I provide.

My site will need to host a copy of the disclaimer ( “I have read the disclaimer” radio button).

Please mention the word “volcano” in your reply so that I know you have read the full description. I may need a number of additional forms and back-end scripting if I like your work.

WordPress Custom Page Temp2
I posted this same project a couple of weeks ago and the programmer I selected never confirmed, so I’m opening it up again.

There are two tasks.

1) I need a very simple page template built. It might be easier to build as a category template. Either way, when this template is assigned, only other posts or pages using the same template will be in the navigation/archive list to the right. I want to set up bio pages for 5 people working in a company. I want the other bio pages to link from that page. I can provide a .JPG file of how the page should work. I’m not too worried about layout issues, I just need the functionality.

2) I need to have a thumbnail image from a post show up in the index page. Take a look at this page: http://www.oregonmedicalweightloss.com/articles/. Under Success Stories an image is being called, but it’s not showing up. I need the image to show up AND I need to be able to control the width of that image so that it can be different from the size of the image in the actual post.

Fix Flash Player Code
I have a flash player on my self hosted wordpress site:

The player for some reason makes the background solid blue instead of white (like on the other pages), it also eliminates the side bar, and makes it impossible to actually write words above or below the player itself.

I need all this fixed…I want the sidebar to show up, I want the background on the pages to be white, and I want the ability to write on the page above or below the player.

WordPress Modifications
I have a site ( http://www.agentpreview.com/ )that I want to implement a blog and except for the sidebar I want to use the current layout, header, footer, and graphics.

* WordPress 2.9.2 is already installed along with a working database with sample posts.
* The CSS must work in all common browsers.
* I want the header modified so there can be secondary dropdown navigation links. Specifically I want to be able to add 2 sub-categories under the link to the blog.
