A body had to be dug up and reburied after Harrogate Hospital released the wrong body to a funeral director, a BBC investigation has found.

The mix-up was revealed in a Freedom of Information request about serious untoward incidents at hospitals between January 2009 and October 2010.

It was one of 51 incidents recorded by North Yorkshire’s hospital trusts.

A spokesman for Harrogate and District NHS Foundation Trust refused to comment on the incident.

The mistake happened between April 2009 and March last year.

When it was realised, an application had to be made to the Ministry of Justice for an exhumation licence to dig up the body and rebury it.

The BBC investigation, which focused on the county’s three hospital trusts, also found other incidents including failure to respond appropriately to child protection issues and incorrect equipment being used during surgery.

Can’t sing, won’t sing? As the BBC researches the musical abilities of the nation, are those convinced they are tone deaf really irredeemably non-musical?

Can’t carry a tune in a basket?

Despite the term, most tone deaf people can hear music perfectly well – they just can’t sing.

And many aren’t tone deaf at all – they simply lack confidence and practice, particularly if their tunelessness was criticised as a child.

“A very small proportion of the population are truly tone deaf,” says Dr Daniel Mullensiefen of Goldsmiths College, co-creator of the BBC’s new musicality test, which explores whether enthusiasm for music – rather than formal training alone – helps confer ability.

“Traditionally, musicality was defined as something that you got from practising for hours in a darkened room. But we want to show it’s a much broader and wider concept.”

So it could be measured by how often – and attentively – you listen to music, how avidly you talk about it, and whether you can tap out a beat.

There’s no singing involved in the BBC test, but it does include tapping beats and sorting clips by genre. So those who claim to be tone deaf may score higher than expected, and feel encouraged to have a crack at making music.

And the easiest instrument to try is the human voice.

Most people can learn to sing better, says Andrea Brown of Morley College, a south London adult education centre which for 15 years has run Can’t Sing Choirs and vocal courses for the tuneless.

“Less than 2% of the students I’ve trained have been profoundly tone deaf. Everyone has their own unique voice,” says Brown diplomatically.

“Some are really quite off-piste at first, and need extra help one-on-one. Most tend to be shy about their voice, and we try to coax more sound out of them. Doing singing lessons is like going to the gym – you’ve got to exercise the right muscles.”

So little time has she with claims of tunelessness that one course is called Tone Deaf? No Way. Other colleges and music teachers around the UK offer similarly titled classes and choirs – all of which provide safety in numbers for those lacking in vocal confidence.

Beginners are taught how to change the pitch of the notes they sing, the same way their voices go up and down when talking.

From can’t sing choir to singer

“There were occasional noises from the bath, but I hadn’t sung for at least 50 years,” says Ian Gorman, who joined a Morley College Can’t Sing Choir several years ago.

“I had a hard time hitting notes and remembering songs. There were some people in the choir with truly awful voices, and others who were average.

“Andrea [Brown] got us doing exercises like yawning before singing, counting up and down scales, and bending forward with a picking-up movement to help reach the high notes.

“Singing badly in a group is great fun. It’s football crowd syndrome – not a single person hits the right notes, but it sounds good. It kick-started me, and now I sing in choirs doing Beethoven and Handel.”

“A lot of the exercises we do with absolute beginners are speaking, not singing. The larynx is moveable and they can pitch their voices. We teach them to co-ordinate what they hear with the sound they make.”

There are also rhythm games, breathing and posture exercises.

And for those harbouring hurtful memories of trying to sing a note played on the school piano by an increasingly irate teacher, a word of comfort – it’s harder to match a piano note than a tone sung by another voice.

“It can help to have a male and a female singing tutor, so the men and women in the class can hear voices with broadly similar tones to their own,” says Brown.

The California-based composer, William A Mathieu, has run tone deaf singing courses for years. He estimates one in 20 people consider themselves tone deaf, usually after being labelled as such in childhood.

He explains why in an audio book on his tone deaf choirs.

“If, in your early student life, you were slightly slower to perceive pitch relationship, it wasn’t long before you were given by your peers, or an insensitive teacher, the identity of ‘the one who can’t sing’.”

The first day is spent sharing stories of hurtful words and shattered confidence. Next, he gets participants to make fluctuating and non-fluctuating sounds, imitating a siren for the first and a dial tone for the latter.

Then Mathieu moves on to note matching. Instead of asking non-singers to match a note sung by a tutor or played on a piano, he asks them to sing a tone, and the tutor then matches it. Hey presto, for the first time, they are singing in tune with someone else.

“An entire class of 10 tone-deafers singing one pitch is a triumph and the excitement is hard to contain,” he says.

Within eight to 10 sessions, simple part-singing can be tried. “Choir fever sets in and catches hold.”

Synthesize a Mini Pluck with Sylenth1

Today I would like to show you a mini pluck sequence. I use this sound quite a lot in my productions, and invented the term “mini pluck”. It is used in several trance songs – especially in uplifting trance – but you can use other genres as well

In this tutorial I’ll show you how to create a “mini pluck downer” type patch in Sylenth. This is basically a pluck arpeggio with a filter being modulated in a random rhythm. I use Ableton Live to create it. It is suitable for giving backing support to other parts of the song. Other words it’s a backing track just give some feel your tracks.


Step 1: Create a Track

Let’s insert a MIDI channel in Ableton and insert the MIDI clip. The length of the clip is three bars. Change the grid to 16th notes and draw some notes on C#4, E, G#, A and don’t leave any silence between them. So as you see the notes is rising on the scale. Duplicate this sequence until you reach bar 2.3. So the sequence looks like the following. This will act as a skeleton for our new sound. The overall tempo is 138 bpm.


Step 2: Tweaking the Sylenth

The riff was quantised and I was ready to go.

Sylenth1 is a virtual analog polyphonic VSTi synthesizer. It is a very basic tool for the trance producers. You can download the demo version from the Sylenth website www.lennardigital.com. Loading up Sylenth and initialize a preset.

Set the polyphony to one because it is just a pluck sound and it not necessary to have polyphony. First, choose the saw waveform on Oscillator 1 and set the voice to two. Put the the oscillator Retrig on. It will light in yellow at right corner of the oscillator section. The Retrigger button can be used to force all voices to start at the exact same location on the waveform every time a new note is played.

Turn the detune knob around 1.81 to give some detune effect. Next add lots of decay to the amp envelope to prevent the volume dying out too quickly, and eliminate the strong pluck so the synth which can be heard in the background. Use a moderate sustain and little bit release also.

You can hear how basic this sounds with one saw. It is not particularly interesting.


Step 3: Filter and Filter Control

Set the filter A to lowpass. You can leave it on 24 – it has more drastic filter effect. Select Input A. Turn the cutoff knob clockwise to around 10 o’clock and the resonance to twelve o’clock. This will cut almost everything.

Give some drive control as well to be more dramatic. Set the filter control to almost the same, and set the resonance a bit lower and the keytrack more than half. The keytrack means that filters cut will follow the pitch of the notes so on the higher notes won’t cut as much as the lower notes. This enables you to keep (or change) the timbre of a sound whenever higher or lower notes are played. And turn on the warm drive at last. You will hear almost nothing because the cut is too dramatic.


Step 4: Mod Envelopes and the LFO

Set a cutoff AB and resonance on one of the mod envelopes. Now we get to the interesting bit and essentially the process that produces the “Mini Pluck” effect. Turn the knobs next to them just little bit. The resonance can be a bit higher. Set the the decay slider to about 1 and the release as well. So you hear that a basic filtered pluck clearly. But it is boring.

Next we set the LFO. The LFO will filter in and out the frequencies during the track so it won’t sound so boring. Load up a cutoff AB on the destination. Set the LFO 1 wave to random, and destination to Cutoff AB. You can experiment with the different types of waveforms, but I usually just stick with the random wave.

Turn on the free LED so that the LFO won’t follow the song tempo. I set the rate very low because I just want the cutoff the filter rise and down very slowly. I set the the gain about half and the the cutoff knob around 3/4 .

You can play and listen how the LFO works. Set the velocity of the misc section to control the cutoff AB, and the resonance as well. Turn the knob around more than 3/4. It is very important because next we will use the velocities to control the filter cutoff and the resonance amount.

Turn on the EQ section, and put the gain about 12 dB between 440-880 Hz in order to hear the mid section more clearly. It is very important to adjust the external EQ because if you apply a standard Ableton EQ the filter in the Sylenth won’t impact on the Ableton EQ and will create unexpected frequency crashes. But if you are mixing the tracks use Ableton EQ to make your sound fit better with other tracks.

Step 5: Velocity and Envelopes

Now we have the basic sound we have to add some excitement, some tension. We are going to use the sequencer to input some velocity columns for our variation to express how the filter and resonance effect is used. To do this use the velocity curves and draw some columns. Remember the velocity now controls the cutoff and the resonance. Lowering the columns make the the filter cut more high frequencies. I use this columns pattern to make the sound more interesting. It’s worth mentioning at this point that you can actually create several patterns to create different resonance/filter effects.

After that I drew an envelope in the clip overview section. Double-click on the pattern clip and and click the “E” icon at the bottom to show the Envelope Box. From the Envelope Editor, select mixer/volume. Switch to Draw Mode by hitting CTRL(CMD)+B and start drawing some envelopes on the Envelope Editor. The result is that the sound will quieter over time and it gives a nice fade out effect to the clip.


Step 6: Adding Effects

Finally I added some effects to give it a larger more spacious feel. I used a compressor as an insert effect in order to eliminate the extreme peaks and volume rises during the sequence and keep the dynamics controlled. The compressor looks like the following:

The next effect is the filter delay. Use all of the channels. The trick is that use different filter settings on each channel and different volume and different delay times. It can be a bit chaotic but chaotic not always bad. Use large feedback to hear the echoes longer. Set the the dry volume at your taste is turned around -5,4 dB.

The last effect is the reverb to make it more trancier. Trance production use lots of delay and reverb. Filter out the low frequencies at the input processing, and make the decay time quite long it gives the impression that the plucks is far away on the mix. Set the dry/wet knob around 20-29%. Take a look at the screenshot to see what worked for me.
