Survey
Hi,
I am needing a basic template of a p o p u n d e r survey site.
I have included the code from one such site. Basically I need a very simple survey that pops up through certain networks and guides users through a series of 4 questions and leads them to an offer page through the survey, with three offers on the first page, and three on the second as well, with an option to go back and forth between those two pages. But not back and forth between the survey questions.
This site should be easily duplicated for different surveys in the html aspect so I can make multiple surveys, with multiple offer pages.
This is a pretty straightforward site, and I have included the vast majority of the coding. I just need it cleaned up and designed.
The site should be 700 x 500 and centered so as to pop up in the middle
