App Like Mach..form

I’m willing to pay $130-200 for this work. It must be done in 15 days or less. Daily updates are required. Most of the requirements are below. There may be some additional small requirement.

The software’s core functionality will resemble MachForm (www.appnitro.com). Which will enable each client site’s Admin user to drag ‘n’ drop elements to create the necessary forms to collect data into a database. The forms should contain validation checks and the ability of the Admin to show and hide form fields to specific users.

The application will have four groups – Patient, Staff, Doctor, and Admin.

Patient – can view and fill out patient’s personal information only
Staff – view and fill “Patient Report” and view only “Exam” and “Queue”
Doctors – view and fill “Patient Report”, “Exam Form” and view/reorder “Queue”.
Admin – create, view, fill and reorder all (where applicable)

The application will consist of three tabs: “Patient Report”, “Exam” and “Queue” tabs

“Patient Report” tab (see attachment form.zip this should give you a rough idea)
Collect patient’s personal information
Should auto calculate age from date of birth field to be used in “Queue” view

“Exam” tab (see attachment img167 & img168)
Includes a series of check boxes that expand options when checked (if necessary). This will cut down on clutter. This will also apply to the complete set of ICD 9 codes (medical billing codes). Only some will be visible but all will be searchable so they can be selected as needed.
This form includes an “arrival time” and “completion time” fields that can be filled by simply touching a button that fills it using the system clock (must also be editable if necessary)
This form can only be altered by the Doctor that filled it out or the Admin.
Must include a “Comments” field at the bottom that time stamps (date, time, and username) any authorized users comments. This enables other users to follow-up on a patient without altering the original form.
Alerts can be set by a doctor to have a follow-up phone call be made on a specific date or a re-examination by another doctor on a specific date. Once these alerts are set the patient should then be moved to the Staff’s patient “Queue”

“Queue”
Is a list of patients that have been assigned and submitted to a particular doctor via the “Patient Report”.
The list is a graphical representation of each patient with a picture/graphic representing the patient (infant, child, young adult, senior adult). It also will include the chief complaint (short summary of why they are seeing a Doctor) age and gender of patient.
The list can be manipulated via drag ‘n’ drop to reorder/re-prioritize the patients
Along the top right corner of the “Queue” tab should display the number of patients waiting to be seen
Please keep in mind that the Staff will also have a “Queue” for follow-up phone calls to a patient as well as re-examinations which will be re-dispatched to an available doctor on that date.
The Admin will also have a “Queue” but it will see the names of each doctor with the list of assigned patients underneath

SproutCore should make some of the drag ‘n’ drop form fields and manipulations easier.
I think this should be enough to get you started. Strong encryption must be used for “PHI” or personal health information (identifying info like name, Bday, sex, SSN)

My client is looking for a developer to design a new site for a non-profit utilizing the Kick(remove_this)Apps social media platform. Job to include:

1) secure Membership management
2) Content management( video/pdf/potheyrpoint repository)
3) E-commerce capability
4) Group calender functions

amongst other capability featured by the Kick(remove_this)Apps platform.

Developer should have familiarity with the platform and preferably some examples of work on the platform.

So they are trying to develop a site that can service a group of physicians in training- specifically for pediatricians working in neonatal intensive care. I run a group in California that provides educational seminars for these young doctors. The aim of this site is:
1) connecting theses doctors via social networking
2) providing educational components to them- videos, PDF, potheyrpoint presentations
3) promotion of our organization and a yearly conference they put on.

I’d like to combine many of these concepts with a much more user friendly interface which the Kick(remove_this)fApps platform provides. What they are looking for:
1) initial look and feel to our site- I think they need some sort of logo or some direction
2) I like to use some of the KickApps exisiting sites as templates

Most of the architecture already includes the wish-list below but just wanted to make sure they can deliver this in the first go-round of development.

This site will be a aimed at the younger physicians working with our professional organization. While a key function will be physician management and social collaboration, the site will require
1) A repository for digital content:
a. User-Generated video content- directly from training programs
b. Digital images(X-rays/Echos/Ultrasounds/MRI)
c. ability for theyb-cast conferences – grand rounds/morning reports
d. ability to include developed educational content- Specialty based Q&A with top physicians in the field.
e. potheyrpoint repository for lectures

2) Most importantly they have a clinical” Manual of Care” that they are trying to promote and have ease of use on the site.. All it is is a pdf. A functional version of this pdf will need to be made for the site on the site( such as easy flip through and they’d like it to be able to be opened on smartphones).

1. I have a web page that is almost completed. I am looking to create I frames or simular so when a link is clicked in the flash menu, only the body/text part of the page reloads not the whole page and flash content.
2. I have contact form that needs to be activated. Also adjust the fields. Both need to provide auto response.

3. I need to have the main page 4 “tabs” that show text under the tab. When another tab is clicked the text needs to change to corespond to that tab related info.

I know how to do this in flash but I do not want flash as when a non-flash user comes the link is unfunctional to use.

Clicked, opens in a new window to eToys.com then copies the text : thetoys to the users clipboard at the same time. So if the user does right click paste or edit>paste can paste the code into the site.

I need 2 custom headers designing for 2 adult websites (1 header per website).

The headers will need to be designed to a high standard, roughly 100-120px in height.

The header will need to be designed including a background gradient, and so that it works at variable widths within the browser, so it will also need to be cut into sections including the HTML.
