Survey Script Process

We need a simple 4 step survey that passes our variables through similar to what that does. Ending up on a final page with various offers and passing our variable through to those links as well.
I would like a template that we can duplicate easily and edit simple text information as well as change out the offers at the end of the process easily.
Would like to start asap.
