1. The patient fills out the fillable PDF online.
2. The patient can print a copy for his records
3. The patient hits SUBMIT and the form is loaded to the database
4. The patient receives a notification email that their form has been submitted.
5. The practice (doctor) receives an email notification that a patient has submitted a form
6. The practice can login to their secure area and download the completed PDF and/or data.
