I need to finish a simple survey page, I’ve started in HTML but I need to make it more professional, i need to make it a php simple survey page. The submit and reset buttons are not working. Then I have a thank you page taht I would like to appear once the submit button is pressed.
The form page could be very similar or the same design as it is now, it only needs to have the functionality working with a professional way. It looks very HTML basic form.
I will need to be able to change the text written if I need.
The project is simple as this, only the form type page, the rest of the design is ok, the logo, the links are ok, it is only the form fields and structure.
I’ll supply the files you just have to fix them.
