Track Form Submissions In Google Analytics

I need a Google Analytics expert to set up tracking for a webform. I have the same form included on 30+ pages with a PHP include. Google Analytics is already tracking traffic/views on my pages.

The form is the main call to action on these pages, so I need to track how many times each form is submitted per the number of visits on each particular page where the form is, not compared with the total visits to the site. This is for testing performance when I make changes on these pages.

For example, here is one of my pages:

I need an easy way to compare the page views of this page with the form conversions on this page and the same way with all my 30+ pages where this form is.

I want to see a spreadsheet view within Google Analytics of all the page views, number of conversions, and conversion rates on these pages at a glance.

Is this possible? Or do we need to build a separate database to track these things?

Can we perhaps use Custom Variables and/or Events in Analytics to accomplish this?
