Create a basic website that can dynamically add/remove multiple jQuery upload prompts by bwobst

I need the code for a website to be able to dynamically add and remove multiple instances of: http://blueimp.github.com/jQuery-File-Upload/ . There needs to be one “Add upload prompt” button at the top of the page (maybe and title)… (Budget: $30-$250 USD, Jobs: HTML5, Javascript, jQuery / Prototype, MySQL, PHP)
