Employee Staff listing search MYSQL PHP by jmc918

I have a page that searches and lists our staff by last name – want to add a drop down selection box that allows search by department, position. Also need to have both search forms on same page – search… (Budget: $30-$250 USD, Jobs: HTML, MySQL, PHP)
