Joomla – create view and form for lookup table by withinmedia

I have an existing Joomla 2.5 component. In the same component I require a new view and a new from. You can create the from scratch or simply copy on existing view & form. The tables is already created, in the same database… (Budget: $30-$50 SGD, Jobs: AJAX, Joomla, jQuery / Prototype, PHP)
