I need a very simple support ticket system to plug into a website. It will have 4-5 fields that the customer fills out and a category for the ticket to be filed under (for instance: billing, tech support, Urgent, System Down, etc)… (Budget: $30-$250 USD, Jobs: MySQL, PHP, Software Architecture, WordPress)
