I have a PHP form that uses data in a MySQL table to craete a PDF document. I want to change the form and the PDF output, so that different data will be extracted, and will be presented differently in the PDF document… (Budget: $30-$50 USD, Jobs: AJAX, HTML, Javascript, MySQL, PHP)
