I need 3 or 4 forms created that will connect to a quoting system. I have seen forms created with jqTransform and like that look. The forms will consist mainly of text input, but I will need drop down boxes, radials, and a few check boxes… (Budget: $30-$250 USD, Jobs: AJAX, CSS, HTML, jQuery / Prototype, WordPress)
