I need to be able to enter a list of headings on a page. Each heading will also need 2 buttons. One button will open a contact form popup which saves the information in a database, and sends it to the admin and submitter… (Budget: $30-$250 USD, Jobs: AJAX, Javascript, MySQL, PHP, WordPress)
