Update form to allow existing data to be edited by crucianp

For Asif: I have an existing form that is used to add data to a database. I also need to be able to retrieve data from that database and edit it with the form. (Budget: $30-$250 USD, Jobs: Graphic Design, HTML, PHP, Website Design)
