I have a DB setup in access 2003. I need to have a form that is a executable so that when a user clicks on it ,the form pops up on their desktop. The form is then filled out by a user and the data recorded back to a MySQL database that is hosted on a G4 database from godaddy… (Budget: $30-$250 USD, Jobs: MySQL, PHP, Software Architecture)
