Form Mail, w/child checkboxes, popups, and captcha in php by Mikeyks1

Here is a example of what I need but I need the second child checkbox to popup with instructions and “Captcha ZDR” added to it. The submit button needs to be grayed out until the agreement box is checked… (Budget: $30-$250 USD, Jobs: Javascript, PHP)
