I need a very simple one-page landing page designed with a php signup form (5 or 6 fields). The form must save data to my server, and redirect the user to a simple ‘thank you’ page. No custom graphics required as I will provide the logo and color scheme for the site… (Budget: $30-$250 CAD, Jobs: CSS, Graphic Design, HTML, PHP, Website Design)
