Bringing a Form from PHP script to WordPress based website’s home Page

I have a wordpress website (link) and another php script sharing same database but in a sub-directory (link).

In that php script I have a form that I want to display on WordPress website’s home page. Please help me how can I do this. Thanks in advance.
