I need a simple poll created that will allow visitors to vote on an item and the results calculated and stored in a database for download via Word and Excell.
The poll should have a field for: Name and Email address.
After submitting the information will go into a database. All submissions can be viewed from an admin control panel where it will show the name, email, ip address and selection that the voter chosed.
All visitors will be restricted to only one vote each. The admin will have access to view, edit or remove each vote submitted and also search through the data.
The admin will also be able to create othe admins is he chooses.
