I am in need of a template from template monster.
i previously bought some templates from there in the range $55-$64.
But i could see the price of this template is a bit higher and i am surprised:).
So instead of buying that from there,I wish to remake it as exactly same,so i can manage the cost of buying the template.

I need a programme that will allow me enter the following and generate a report that can be graphed.

Date of Service:

Location of Service: – This should be a drop down menu that will populate itself when details are entered (You know the kind that will allow you to enter ur own choices and also allow to select from a dropdown the ones that has been entered by you before)

Type of Service: – This should be a drop down menu that will populate itself when details are entered (You know the kind that will allow you to enter ur own choices and also allow to select from a dropdown the ones that has been entered by you before)

Special Guest: – This should be a drop down menu that will populate itself when details are entered (You know the kind that will allow you to enter ur own choices and also allow to select from a dropdown the ones that has been entered by you before)

Sermon by: – This should be a drop down menu that will populate itself when details are entered (You know the kind that will allow you to enter ur own choices and also allow to select from a dropdown the ones that has been entered by you before)

I want to be able to search by Date of Service, Location of Service, Type of Service, Sermon By, Sermon Title, and Special Guest.

– The Search field for Date of Service should be a calendar.

– The Location of Service, Sermon By, Sermon Title, Special Guest and Type of Service should be an auto complete field. That is, it should check the database and bring options close to whatever is type and also have a drop down to select the available services types already typed
The search result should bring all the details shown and I should be able to play the mp3 sermon attached to the message.

I need to be able to generate report that can be exported to excel and graphed.
– Report to show a graph of attendance (Male, Female, Children & Total) against Date of service for a period of time (Specify start and end dates)
– Report to show a graph of attendance (Male, Female, Children & Total) against Service Type (Specify start and end dates)

All the forms, reports and results should have this header below so I can print:
