Calling it more than once on the page of file upload fields not working as we expected

i have created file upload filed it is working as expected, but calling it twice on the same page is not working(the second file upload is not working), I’m sharing my sample code here, kindly help me on this? – Calling the HTML forms twice is causing the issues for the second form file upload
