This project requires knowledge of PHP and MySQL.
Essentially I need a PHP script that will INSERT/UPDATE a MySQL database with information from a form that is created dynamically (‘on the fly’) with PHP.
The form has already been created, the part that is required is the script that will run through each form input field INSERTING/UPDATING as required.
Please see the attachment for full information of the requirements and a breakdown of the database tables etc.
This represents one of the last hurdles I need to overcome with this site so a quick completion is required. Please note: there will be no access given to my MySQL database but all necessary information will be provided.
