Simple Squeeze Page Maker Needed

Hi,

I need a simple “squeeze page maker” that will simply take the Aweber html code and generate a Mobile optimized html page.

Here is a sample of what I need…

and this generates the following html page…

Here is the same thing (only difference is it generates slightly wider email & name fields…

This should be fairly simple to code for someone that knows what they are doing and familiar with this. It should be a very fast coding job. I guess, the script would simply be embedded in an html page and then generate the optimized page in a separate html page.
