i need an exact match to the animation of this intro applyed to the pic i have for my intro

the 3 lines on that site the animation has to be the same for the intro i have
i have all files of the design of the intro page, just need animation for my page
i need a script when clicked enter ands there are 2 checkboxes if clicked skip for a week, then the ip goes to some list that skips the intro for that person for that ip for a week, if its a month then he skips getting intro for a month
the animation should be as follows:
the 3 lines at the top slide down from top to bottom, then the enter logo with the enter sign on it has some sort of efect
then the 3 lines after enter are exact match to the animation lines on the website i provided here
then the character and shadow should be about same as the animation on the site i provided.
if it cant be done i cant pay you.
