Web application that allows Users to record the subject and source of calls (and a handful of other fields) on a single page that refreshes a clean sheet on submit. The user’s page needs a link that allows them to enter total hours worked that day and a reminder on logout if they did not fill it in yet.
It also must include an administrative backend that allows:
1. Generating reports. ROWS-categories, COLUMNS-date range(variable), CELL-total calls matching that category in that date range. A second/ appended report that calculates the hours dedicated to those calls is also required
2. adding/deleting categories
3. Download of database as XML or CSV file
Server Requirements
PHP 5 (Specify in bid if you’re going to use any 3rd party scripts to assist in development and what they are)
MySQL
