Facebook has changed the layout of the custom tab pages just recently. we need you to make the form fit within the window

you would need to move the 2 right side fields and move it on the left side so that all fields are below each other.
we can provide PSD file if needed.
we can make you admin in facebook so you can get the job done or you can update the attached HTML code and we can update it from our end.
