I need a signup/billing page for Users to signup for email/subdomain/ftp accounts. Would need to have them created in Cpanel. Here is details of what I am looking for:

Signup Page for Client:
Customer would signup for FusionMail Email Account with packages
Payment would go though paypal
Required Information from clients:
Email Details:
Username
Password, Confirm Password
Alternate Email
Secret Question (Typed Out)
Secret Answer (Typed Out)
Personal Details:
FirstName
LastName
Birthday (Drop Down Menu)
Gender (Radio Button)
Town/City (Drop Down)
Province/State (Drop Down)
Country (Drop Down)
-Select Package (Default Would be Free)
-Places Coupons for discounts
-Payment method would be Paypal only
-Billing Cycle starts on the date of signup

Billing Page:
-Upgrade/Downgrade Packages
NOTE: Downgrade will only happen in the next yearly billing cycle
NOTE2: Downgrade will REMOVE necessary packages (removes ftp access, or blog and sub domain) on next yearly billing cycle
NOTE3: Near Renewal date, will auto send email for reminder
NOTE4: If Upgrades, would adjust payment so that matches end of billing cycle

Forgot/Reset Password Page:
Would ask to verify the account with secret question and answer
if can’t verify, then can email to admit for assistance

Signup Page for Sponsors:
sponsors would be able to select package (1 year payment)
Sponsors be able to either upload or use URL for banner ads (small or large banner)
sponsors be able to add text line for ads
Sponsor Login page:
sponsors be able to upgrade packages
sponsor be able to downgrade packages on next billing cycle
sponsor be able to see status of banner ads (impresses, clicks, etc) on monthly bases per banner ad (large or small)

Admin:
-Block certain username from being created (root, support, admin, postmaster, etc)
-Create/Edit/Delete Packages
-Create Coupons (Discounts for packages)(have expiry)
-Package Options: Email Account, FTP Access, Sub-Domain, Word Press Blog (Auto Installed to sub domain), pricing, term
-see total income, and graphs of usages (signup, sponsor signup, coupons used)
