I have 2 contact type forms that I want some modifications made:
1) On both forms I want to replace the javascript captcha with a PHP captcha.
2) On both forms I want to have the submit image button replaced with a submit button that uses CSS with a background image.
3) On the second form add a checkbox (e.g. Agree to Terms of Service) that must be checked.
The first form can be found at: http://www.agentbios.com/contact/ and it includes a sample button that incorporates the format (image and CSS) I want to use for the new CSS submit button.

When placing your bid I would like for you to provide links to forms that use a PHP captcha like you would implement for this project. Also, if you have done any CSS submit buttons with a background image you can provide links to them.
