Need to add extra fields to my form. The info will be stored in my MySql db.
I need:
– Apt or Suite
– City
– Zip Code
– Day Telephone
– Date of Birth (mm/dd/yyyy)
– Gender (this should be radio buttons to select Male or Female)
– Email Address
– Validate Email
