Need a data form written in HTML (PHP) to handle user input for food product data (about 20 fields)that is stored on an SQL database you will build on my server. Must be able to easily download data into Excel format. Form must also have “add” buttons that link to secondary page to “add another” or “finish”, then take them to a confirmation landing page.
