I need an on-line form that looks like TAGS.pdf that sends a pdf that looks like TAGS.pdf to my email and fax.
1. When the email and fax arrive they look exacly like the TAGS.pdf with the customer info.
2. There must be a check box – That they accept the terms
3. Must sign with name and initial box
4. Verify email below before send and must match email above.
5. This is a contract.
6. Install on websites.
