I am interested in a script for my website where I can generate quizzes. I need people to opt in before being able to receive the quiz questions. I would also like to have my logo information and branding in the quiz. Also, the quiz needs to populate the answer report based upon how the questions are answered. Finally, I would like the last page to be the results in a report style format with an offer at the bottom.
The quiz will be used on my wordpress website.
