Capture Page

I need to setup a Capture Page with:

1. a brief message/question and
2. an entry box for people to submit their question and
3. include my autoresponder form (for Name and email)

Once they click submit, the question(requirement 2 above) should automatically be emailed to my email’s inbox.

This should be an easy project if you know what you’re doing.
