Hello there! I’m in need of a PHP programmer to assist me in creating a single page web form. The web form needs the following options:
-Display categorizes via drop down box (pulled from mySQL database)
-2 text boxes (with word limits)
-Ability to upload multiple attachment
-Ability to add multiple website addresses (textbox)
