I am in need of a lightbox form that will
1. Pop-up lightbox form from a link on the main page
2. Record the submitted information into a mysql database
3. Display the submitted information from the database on the main page once the form has been submitted without refreshing the main page.
The lightbox form must also have a “cancel” link that will close the form if the user does not want to submit it.
