We have javascipt code that we place on our clients websites and it feeds data to the website. We want to have it where when someone clicks on the information, a screen comes up and asks was this information helpful with a yes or no button. If yes, then a form comes up and asks what they found helpful and they will put in their information. The information will then be emailed to the client. If no then it says thank you and the box goes away.
