I need a high quality mobile optimized web page (1 page) that is designed to be viewed on iPhones and Androids. I need a simple and nice looking design.
The home page needs to have a form with 4 text fields on the left side of the page. At the bottom of the fields there should be a submit button. On the right side should be a place for text. There should also be a footer with 3 dummy links.
When the user clicks submit the website will redirect to another webpage and pre-populate the fields…
