I need a 2 part contact form.
It should all be written in PHP
The first page will have some form fields to fill out. When the user fills it out the info will be sent by email to me and put in MySQL database.
They will then get the second page with additional fields to fill out. When they fill that out the info will be sent to me by email (with the first info included also), and added to the same database entry so it will be more complete.
