I need a form made exactly as per attached – Must look the same. Form will go on a wordpress website. You will also need to set it up on the homepage ( right side of the content ) on the WordPress site. Must have empty field validation check. On submission Admin and User should get emailed a copy of the form data. You can use an existing WP plugin if it works for you.
