Hello,
I need to add a form to my page so at the end it looks something as:
http://www.royalcarerx.com/fill-your-rx/refill.php –> page 1
Then for another page add a form with button upload so users can choose file which to upload. (so they can upload their prescription).
Results can be saved in db and would be good if an email notification is sent out if any user completed either of the forms.
