This is a very small project for an xhtml developer with knowledge of working on forms. Please find attached a screenshot to understand the project fully BEFORE placing your esteemed bids.

I have one page of my site which needs to be fixed for 5-6 things. all the elements to be fixed are indicated with red marker in attached screenshot.
