I need a contact widget placed on the left hand side of every page, similar to the widget titled “Schedule Consultation” here > http://www.orkin.com/commercial/about/
It should be a simple HTML email form. It does not need to go to a database. I will provide you with the email address.
It NEEDS to he a PHP include on every page – the include should point to one file, so that if i have to edit this file one day i do not have to do it on every page.
IE: <?php include ‘mail-widget.html ?>
I don’t care how you accomplish this, as long as it is easy to edit and it looks good. I will give you the text info that goes in the widget.
