From Management Script Clone

I need a form management script clone similar to the Machform script (http://www.appnitro.com/)
Needs to work without any third party involved! Needs a front end and a backend to manage the forms. Saves captured information from forms to a MySQL database. Need to be able to download database information to an Excel file.
Need all features found in the MachForm script.
