I need very straight forward squeeze pages built for a new business that I’m helping out with.

We are looking for seniors with medicare and arthritis. All the questions in there form are needed in our form. The layouts should be very unique to each design, but one of them at least can be very similar to the page shown above. We will preform some A/B testing with these pages.

The 3 pages need to look entirely different.
We’re trying to get a high % of conversions for this product so make the form attractive for people to fill out.
It all have to look incredibly professional.
I have a few stock photos but I’d prefer to see you all be creative and find images yourselves.
Besides the 3 pages, you’ll have to make a generic thank you page for each one as well. The squeeze pages need to display our phone number as well.
