Need template (layout) of support script changed to have same layout as my new website. There are 4 template files that need to be edited: footer, header, register and index. (see attachment!)

I started the job but it’s not very good 🙁 Now I need a very experienced coder with great knowledge for HTML, CSS, PHP and MySQL to create/copy the ‘real’ layout to this supportscript.
