I have created screen shots of how I want the form to look and feel into our existing site southfloridasecurityjobs.com
I need the form to be able to email us once the form has been submitted with all the information and stored into a database.
I need the fields on the first form all fields need to be required and the fields listed for employment history to be required.
On the app4.jog file you will see that the applicant should be able to enter additional job history on the next page or click next to go to the next section of the form.
The form should flow just as the pictures show 1,2,3,etc..
The form needs to be implemented into our exiting website template just as the picture shows.
I want the form to look EXACTLY as I designed it in the screen shots.
