We need someone to minimize the size of the form so we can embed the PHP form code into a website page.
We need the form PHP code delivered so we can easily insert the code in our website page. Currently this form is standalone and we have to link from the website page to the form.
So basically we only need the form functionality from steps 1 to 5 and really compact/minimized size.
Remove all background colors, width, borders, etc, minimize some elements within the form like form fields, make images in steps 1 and 2 smaller and “keep the current functionality”.
The space we have on our website allows a width area for 650 pixels so the updated form should fit in this area.
