I currently have a form that is in Microsoft Word. I need it to be converted into a PDF file that may be filled out online. Then once the user is finished with the form, they may save it and easily email it back. If possible, a user interface to make sure they save the information would be good, too. They also should be able to print the form, fill it out by hand and fax it in.
One of the forms is an extensive, eight-page health questionnaire (200 questions). It is important that each question have the appropriate fillable area to answer the question properly.
